Nature of benefit<\/strong><\/span><\/td>\nIn addition to government funds for surgery of needy children, a maximum of Rs. 50,000\/- per beneficiary will be provided, but the subsidy will be given to the bank account of the respective beneficiary.<\/span><\/td>\n<\/tr>\n
Eligibility criteria<\/strong><\/span><\/td>\nChildren up to 6 years of age who have undergone surgery for a serious illness.<\/span><\/td>\n<\/tr>\n
Required documents<\/strong><\/span><\/td>\nCompleted application form, certificate from the health department regarding surgery for a serious illness, Aadhaar card, proof of local residence (e.g. ration card), Poverty Line Number and certificate or income certificate from the Tehsildar, copy of Gram Sabha resolution if in a tribal area, copy of bank account passbook.<\/span><\/td>\n<\/tr>\n
